Registration

We require that all new patients complete the Rusnak Family Dentistry new patient registration prior to being seen for their first appointment. There are several ways to complete your new patient registration according to your preferences. See below to find the most convenient registration option for you!

Submit Electronically

You are able to complete your dental registration electronically by clicking on the link below. If you choose to do this, your registration will be uploaded straight into our system and all you will have to do when you arrive is sign for the paperwork you submitted. Note: You must have a scheduled appointment for the paperwork to submit correctly.

Print At Home

Feel free to print our new patient registration from the convenience of your home, complete, and bring it with you to your first appointment. You can download the registration forms below. This will save you from having to arrive early to complete your paperwork in the office.

In Office

As always, you have the option to complete your new patient registration when you arrive for your appointment. In order to see you for your dental appointment on time, we do ask that you arrive 15 minutes prior to your scheduled appointment if this is the registration option that you choose.

Updates

We are required to update your contact information and medical history that you have provided to Rusnak Family Dentistry every 2 years. If it has been 2 years or more since you last updated your information, you will be asked to complete a short registration update when you arrive for your next dental appointment. If you know that something has changed since then, you can electronically update your patient file by clicking below and changing your information. Note: If you have not registered for our online patent management system you will be required to setup and User ID and password.
